[Catalyst] catalyst/helper .t files patch

It bugs me a little when working with the auto-generated .t files,
that app.t, model.t, controller.t files aren't all formatted exactly
the same.

This patch makes sure there's the same line spacing between different
parts, and that all the use_ok() calls are within a BEGIN block

It's for file trunk/Catalyst/lib/Catalyst/Helper.pm

Hopefully this isn't too anal a request!
