I think you forgot to push your single commit version.  Second your "fail" mode dies out poorly:

      $ perl -Ilib t/postgresql-rename-table-and-field.t
      1..10
      You tried to plan twice at t/postgresql-rename-table-and-field.t line 16.
      # Looks like your test exited with 2 before it could output anything
