[Dbix-class] Surprising behaviour of add_to_foo method for many-to-many relationships

One of our applications using DBIx::Class was recently bitten by the surprising (to me!) find-or-create behaviour of add_to_foo methods generated by many-to-many relationships.  I filed a bug report <https://rt.cpan.org/Ticket/Display.html?id=124501> but was also curious if anyone else is surprised by this or has encountered it in the past.
