[epo-core] Online Membership form/New Member

Hi all

Well it is several weeks late and a lot more effort than I imagined  
but the online submission form for Membership - the short application  
is up on the EPO website (which has also had a re-design to look a bit  
more modern and modern business friendly).

As always I would appreciate some comments and as I mentioned in my  
last email once the form is up there we will have to start sending out  
membership calls, these will go out today.

When they fill out the form and request membership we will have to  
decide if they can be members, this wll be done on a matter of board  
decision, as outlined in our rules and we have one to decide upon.

Eden Cardim has requested Ordinary membership votes for aye/nay if you  
please.

Please note that a new list will go out to new members asking for  
details, some of which will be published online in the list of members  
available in a members-only area of the website, in which we will also  
publish the companies finances - to remain transparent in our dealings  
to our members. The Details requested will be:

*Nickname:
*Real Name:
Address:
Email Address:
*Type of Membership:
Date started:
Renewal:
Membership Number:

(*star indicates data that will be published in a list of members in a  
Members-Only area of the website)

Other data I will store is the amount paid, any donations etc., these  
will not be made available except to audit/annual review.

In order to keep some anonymity in the finances members donations/ 
subscriptions will be by membership number which we should give -only-  
to the member and not publish except at their express written  
permission.

If anyone has any more thoughts, or wants to revise this, please let  
me know them asap. I am just going ahead until someone screams stop :)

But we must vote on Eden Cardim in the next 24hrs please.
