I've checked in my doc changes.

I've consolidated the FAQs into the cookbook - as there was some
duplication between them - so there's no longer a FAQ section in the
main HTML/FormFu.pm docs, just a link to the cookbook.

I've removed the CAVEAT section, as the cookbook now has a
introductory YAML section.
