[html-formfu] exporting nested_name to ExtJS

Done for FieldSet, including test.
Not possible for block, due to not exporting Block.
Not yet done for Repeatable as I'm still thinking if I can generalize and CPAN 
our Repeatable module.

The changes in Select element where not from me. I removed my name from 
Changes file.
