[html-formfu] now on github

I've imported HTML-FormFu into github.
I'm hoping that its nice fork / pull request interface will help me
get people's contributions added more quickly.

I've removed everyone's svn commit privileges from the googlecode
project, so that no further changes can be made to it.

I'm still in the process of moving H-F-Model-DBIC, Catalyst-C-H-F, etc
into github - they should be all done within the next day.
