[Number-phone] Number::Phone 3.4002 released

This quarterly update is on its way to the CPAN. It includes:

* new US and Canadian area codes;
* updated data from OFCOM and libphonenumber;
* Number::Phone::UK now uses a stand-alone database instead of
   one embedded in the Number::Phone::UK::Data module;
* Number::Phone::UK should no longer crash if your code forks;
* you can optionally slurp the UK database into memory, exchanging a
   very long startup time for faster access once it's loaded.

The next scheduled update will be in early December.
