Bingo!

It's useful having a checklist in one place like that. I've now solved
my problem.

Incidentally, the UTF-8 problem wasn't showing when I ran my app as a
standalone catalyst_server, but did appear when I used FastCGI, hence
why I wasn't seeing the problem on my dev box.
